How do I submit a prescription?

Prescriptions and enrollment forms can be mailed to:

In accordance with Board of Pharmacy Regulations, only a physician may submit a prescription through fax, phone, or ePrescribe. Patients may mail original, hard copy prescriptions to Eagle Pharmacy. Eagle Pharmacy will fill medications upon receipt unless otherwise noted on the prescription to hold/profile by the patient or physician.

What happens once I send in the enrollment form and my prescription? How do I check the status of my order?

Once we receive your completed enrollment form and prescription information, we will process your order and your medication will be shipped directly to your home within 5-9 days as long as no order processing issues arise. You can check the status of your order at any time by calling 1-855-799-6834 or by emailing BI@eaglepharmacy.com (email is monitored Monday through Friday, 8:00 AM–5:00 PM ET, excluding holidays).

How do I order a prescription refill?

You can refill your prescriptions by calling us at 1-855-799-6834. Customer Support representatives are available 24 hours a day, 7 days a week. Eagle Pharmacy’s hours of operation are Monday through Friday, 8:00 AM–5:00 PM ET, excluding holidays.

You can also elect to refill your medication through the Automated Refill program. Each month, your medication will automatically be shipped directly to your home and the credit card on file will be charged prior to shipping. Patients are responsible for all shipments and co-pays while enrolled in this optional service. You may opt in or out at any time by speaking with one of our Customer Support representatives at 1-855-799-6834.

Insurance/Medicare/Medicaid

Can I use my commercial insurance coverage if I enroll in the MOBIC Door-to-Door program?

No. All Door-to-Door prescription transactions will be by credit card only. Everyone pays the same cash price of $30 for a 30 tablet supply, or $75 for a 90 tablet supply of MOBIC, with no insurance required.

What if I have Medicare Part D or Medicaid insurance? Can I still enroll in this program? How much will I pay?

If you have Medicare Part D, Medicaid, or a similar state or federally funded medical assistance program, you will pay a cash price of $30 for a 30 tablet supply, or $75 for a 90 tablet supply of MOBIC. All Medicare Part D orders are processed without the use of insurance and cannot be applied to Part D true out-of-pocket (TrOOP) costs. State and local taxes may apply.

Am I eligible if I don’t have any insurance? How much will my MOBIC cost?

Patients have a choice of two payment structures: 1) Patients can enroll for a 30 tablet supply of MOBIC for $30, or 2) Patients can enroll for a 90 tablet supply of MOBIC for $75. All patients, upon initial enrollment, will receive an additional free 30 tablets of MOBIC. Enrollment includes the fee payment for the initial prescription.

  • Osteoarthritis (OA)
  • Rheumatoid Arthritis (RA)
  • Juvenile Rheumatoid Arthritis (JRA) in patients weighing 132 pounds or more

MOBIC, like all prescription nonsteroidal anti-Inflammatory drugs (NSAIDs) increase the chance of heart attack or stroke that can lead to death. This chance increases if you have heart disease or risk factors for it, such as high blood pressure or when NSAIDs are taken for long periods.

Do not take MOBIC right before or after heart bypass surgery.

Do not take MOBIC after a heart attack unless your doctor tells you to. It could increase your chance of having another heart attack.

NSAIDs including MOBIC can cause bleeding that can be serious and result in death. This can occur at any time but your risk of bleeding is higher if you take higher doses of NSAIDs or take them for a longer time. Your risk of bleeding also increases if:

  • you have had bleeding problems in the past,
  • you have had stomach or intestinal ulcers,
  • you have liver disease
  • you are older age
  • you are taking certain other medications such as corticosteroids, anticoagulants, or “SSRI’s”
  • you drink alcohol or smoke

NSAIDs like MOBIC should be taken exactly as prescribed, at the lowest dose and for the shortest period of time needed for your treatment.

Before taking MOBIC, tell your doctor about all of your medical conditions, including if you:

  • have a history of ulcers or bleeding in the stomach or intestines
  • have high blood pressure or heart failure
  • have liver or kidney problems
  • have asthma
  • are pregnant or plan to become pregnant
  • are breastfeeding or plan to breast feed

Tell your healthcare provider about all of the medicines you take, including prescription or over-the-counter medicines, vitamins or herbal supplements. Some medications may work differently or cause serious side effects if you are taking an NSAID medication.

NSAIDS can cause serious side effects including:

  • new or worse high blood pressure
  • heart failure
  • liver problems including liver failure
  • kidney problems including kidney failure
  • low red blood cells (anemia)
  • life-threatening skin reactions
  • life-threatening allergic reactions

Other side effects include stomach pain, constipation, diarrhea, gas, heartburn, nausea, vomiting, and dizziness.

Get emergency help right away if you have any of the following symptoms: shortness of breath or trouble breathing, chest pain, weakness in one part or side of your body, slurred speech, swelling of the face or throat.

Stop taking MOBIC and call your doctor right away if you get any of these symptoms: itching, yellowing of your skin or eyes, vomiting blood, there is blood in your bowel movement or it is black and sticky like tar, unusual weight gain, skin rash or blisters with fever, or swelling of the arms, legs, hands, and feet.
